Position Summary: The Wellington Hospital Rehabilitation is currently recruiting for the position of Discharge Co-ordinator. The role of Discharge Co-ordinator is to assist in the discharge/transfer of UK based patients who require multi-agency working to ensure safe and appropriate transfer of care.

General
Responsibility:

The position requires the successful candidate to liaise with rehabilitation reservations department in order to gain pre-admission information on the patient to identify possible difficulties that may occur in transfer of care and to liaise with the interdisciplinary team, consultant, patient and family in order to formulate the discharge plan. The successful applicant will also be expected to assist in carrying out the discharge plan by making appropriate referrals and following them up to ensure safe discharge of patients.

Requirements The successful candidate will have First Nurse Level 1 or be an Allied Health Professional with specialist experience in rehabilitation. The successful candidate will also have knowledge of current legislation relating to community care and patient’s entitlement relating to funding for continuing care as well as knowledge and understanding of funding provision from health insurers. A good standard of written and oral English is essential. The successful candidate should also have good interpersonal and communication skills as well as excellent customer care skills.
